Have you been off work and you're now wondering what happens next?
- You do not require a doctor's sicknote for any illness lasting seven days or less
- If you are sick for seven days or more you require a Doctors sick note
- If you feel okay to go back to work but cannot do your job to the full extent you may need a 'Fit Note'
How do I self Cert for 7 days?
Your Employer may require you to complete a self-certification form (SC2) which will be available via the company or the government website HMRC Self Certification Form
How do I get a sick note?
Once you have gone over the time period of 7 days you will require a Doctors sick note.
Please contact the surgery on 01902 722359 and ask the reception team for a sick note.
You will then be sent a text message to respond to please include the following in the response:
- The day your self certification ended
- A brief description of your current situation
- The time frame you are requesting off
Please kindly fill out this text message as soon as possible otherwise this will cause delays in creating the sicknote for you.
Once we have received a response this is added onto the next available GP query for them to action as soon as this is done you will receive a message back with your sick note attached.
What if I don't have a smart phone to do this on?
Please do not worry if you do not have a smart phone as the team can work in other ways to arrange all the above to happen for you
Statement of fitness for work - 'Fit Note'
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.
